Smart Start Initial Assessment™

The Smart Start Initial Assessment is intended for those of you who might be in the process of trying to find answers to an undiagnosed health concern or recently diagnosed with a complex, serious, or life-threatening illness. It is also a great option if you are anticipating an upcoming surgical procedure, managing one or more chronic health conditions, seeing multiple specialists, taking numerous medications, needing assistance with senior care planning, or coordinating end-of-life care.

This package includes in-person consultations, assessments, analysis, planning, and recommendations based on a comprehensive health history intake and medication review. Our advocates take the time to conduct a holistic review of the mind, body, spirit, relationships, and environment. This provides the opportunity for an in-depth discussion of all aspects of your health history, allowing us to identify any areas of health that may warrant further investigation or discussion with your healthcare providers and create a plan and identify resources uniquely tailored towards your individual health goals.

The Smart Start Initial Assessment serves as the foundation for subsequent professional advocacy programs. It is the starting point for clients seriously considering independent advocate services. It can also be used as a template for self-advocacy for those who prefer to do it themselves but may need a little guidance and assistance discovering various options.

As a client, you will receive an individualized action plan containing potential next steps, suggested options for resources and referrals, an organized health summary resource document, and a review of how a professional healthcare advocate can help implement that plan. You will learn specific ways in which private advocacy services can improve both the patient and caregiver experience through education, communication, and connection to resources serving as a guide to help you reach your health goals.

  • 1- in-person consultation at a location of your choice (up to 90 minutes)
  • Comprehensive personal and family medical history intake
  • Basic medication, vitamin & supplement review
  • Health summary document perfect for sharing with your family and providers

Comprehensive Assessment Includes:

  • 1- in-person consultation at a location of your choice (up to 2 hours)
  • Comprehensive personal and family medical history intake
  • Basic medication, vitamin & supplement review
  • In-depth review & analysis of gathered health information
  • Identification of key concerns based on comprehensive assessment and analysis of health history
  • Recommendations for any areas that might warrant further investigation, intervention, or discussion with providers
  • Identification and review of potential resources, referrals, and next steps
  • Health summary document perfect for sharing with your family and providers
  • Personalized action plan
  • 1- in-person followup consultation and action plan review (up to 1 hour)
